MIND THE GAP:

The Role of Nursing and the Interdisciplinary Team

Keynote Address: Margot Wilson and Delia Cooper

Will discuss gaps in continuity of care and the importance of the patient voice in health care redesign. 

Panel Session:  From the domains of education, research administration and patient perspectives

Discussions will focus on the experience of care by an individual with his primary care provider and longitudinal continuity of care and will explore the 3 subthemes of:

• Informational continuity
• Relational continuity
• Management continuity

Concurrent Sessions and Poster Presentations will showcase research and projects of scholarly inquiry from clinicians, nurse researchers, administrators and students. 

Pre-Forum Workshops with provide interactive sessions to explore continuity of care
